LexMedia members are required to fill out proper paperwork when using LexMedia Assets. Please take a moment to review the forms listed below and see if you need to fill out a form and return it to the LexMedia TV Studio. Some forms can be filled out online. Â Other PDF forms can be printed out at home, filled out and then mailed or dropped off at 1001 Main Campus Drive, Lexington, MA 02421.

Â We ask that people writing about LexMedia use the graphics and descriptions on this page when referring to LexMedia in articles and publications about our organization.
